A large gear attached to the tabletop and a smaller gear that meshes attached to a crankhandle with indexed pins or stops. Like a bicycle pedal/gear assembly with a welded rod connecting the crankshaft, as it were, lenghtwise undernath the table.

The slickest one I have ever seen was written up in WOODWORK (#87, June 2004) in an article by Roger Heitzman (Page 68), called THE ULTIMATE LAYOUT AND ASSEMBLY TABLE.
As the article title implies, this is not a work bench, but the same mechanism could easily be used. The hydraulic lift alone cost him $600, and welding is required to build the supporting frame.
This may be more than you need, but as I said, it is the best and most versatile design I have run across.

Yeah, I saw that too, bloody great idea. I wish I had one ,but by the time I get around to making one I'll be retirement age.

When at the Philly furniture show this year, I ran into Jeff Noden, who makes an adjustable bench. It really works, though I didn't buy one. He is in Trenton, New Jersey. You can check him out at http://www.geocities.com/adjustabench
