I have a Delta 14″ band saw in which a 4″ extension kit was recently installed. I have gone through the techniques shown in Duginski’s book and I think the wheels are parallel and the 1/2″ blade is at the correct tension.
When I run the saw the blade moves forward and backward once each revolution. That is to say it moves towards and away from the operator by about 1/8″.
Can you think of an explanaton? Do blades ever warp in some way?

A small amount of back and forth motion is common and harmless, but a movement that large isn't normal, though the blade may still be usable for rough work like ripping.
If the motion occurs with one full revolution of the blade, then I would take a look at the weld. If the blade shop wasn't being careful when joining the blade ends, they could have joined them with a dog leg kink at the joint. You should be able to place a straightedge, gap free, on the back edge of the blade over the weld. A kink somewhere else on the blade is possible but very rare. The only fix, for either cause, would be to replace the blade, which the shop should do for free.
If the blade is moving back and forth with one full revolution of the saw's wheels, then I'd suspect a warped or bent wheel or a tire in poor condition.
