What is an acceptable tolerance for miter slot being out of
square to the blade? My saw is out .003 thou. front to back.
Do I need to get it closer?

It depends on how finicky you are. My saw manual states that less than 1/32" is acceptable. So, by that definition you're good.
Do you get any (double) tooth pattern on rip cuts? No? then you're done. Yes? Then .003 is not good enough.
On my dial caliper I get almost no deviation from front of blade to back. Make sure you are really tight to the miter slot as you slide back and forth, and that you are measuring the same tooth (the body of the blade, not the carbide chip) at the front and back of the blade rotation. Until I adjusted to less than .001", I had tooth swirl marks.
