There's two ways to do it. In wood, cut a hole through the door. Make it big enough to get your fingers through it and wrap around the hole's edge so you can pull up. Then add a plate under the door to close off the hole, while still leaving depth for your fingertips. In metal, you can buy hatch handles which fold flush. You can get them at the usual woodworking hardware sources, or in marine supply sources
