Looking for phenolic surface plywood
In the #198 issue of FW Stuart Lipp made a strong point for phenolic surface plywood as material for jigs etc. I have now spent the whole day trying to find a supplier who will selll me a single sheet in Southern Ontario, Canada – unsuccessfully. Is there a Canadian woodworker out there who will give me a hint?

Sources in the U.S. are scarce, too. Rockler and Woodcraft usually have small pieces, 24x30 or so, but shipping to CA may be an issue. Also, be aware that there is a "construction grade" used for concrete forms and such - construction grade ply with a phenolic film glued on.

I get regular E-mails from http://www.woodpeck.com and they do fairly regular sales of the ply you are looking for. They take advance orders and buy in bulk. I just got one not to long ago. Reach out to them. They sell it from time to time.
High Density Overlay Plywood
Unlike Ralph I have good experiences with Formwork Plywood. What I have been purchasing is 13 plys with no voids and the surface is very durable. I actually built all the countertops in my shop using it. One of the main reason I like it is, nothing sticks to it. So glue and epoxy pop right off. Of course the bad side of that is glue won't stick to it, so everything has to be nailed or screwed. I know it is available in other thickness but I have only found it in 3/4" thickness. I think you should try it. I have been paying less than $40 a sheet for it, which makes it quite a bit cheaper than other alternatives. You will have to find a construction materials supplier to buy it, you won't find it a Lowes and HomeDepot.

phenolic plywood
there is a place in Roseville, Michigan right by Detroit called McCausey Lumber. They sell it by the name of 'concrete form plywood. Its avalable in 3/4 in. 14 ply and 1/2 in. 9 ply and around $100 for a 4x8 sheet. I used it for my outfeed table behind my table saw and love it.
