Why not buy a good brand of screwdriver and replace the handle? Klein screwdrivers which can be found at electrician's suppliers and at Home Depot in the electrical section have good quality blades.

Jason you might try and find Jim Kingshott's book making & modifying woodworking tools. It has a chapter about making screw driver's. Hope this help's Jerry
Xcelite brand interchangeable screwdriver blades have a pair of very stout fins for transferring torque to the handle, which should ease the job of designing and building a custom handle. I don't know if they meet your quality standard. I do know they're ubiquitous in the computer / electronics industry.
