The May/June 2008 issue of FWW had an article on phenolic plywood that listed sources, one of which is in NY:

Call them and see what they have to say.
Woodcraft was also listed as a source.
You can access the FWW online if you are a member or by signing up for the 14 day free trial.

How about Baltic birch plywood? I went to Kuiken Brothers and was surprised that they didn't carry it. They focus more on lumber for construction rather than woodworking.

==========
I found a Paramus lumberyard that sells Baltic birch.

Woodcraft works for me. I didn't realize there was more than one location, and although they don't have a store in NJ, there are several in PA. We pass close to one of them when we visit my in-laws. Thanks.

Note, too, that there are at least two different grades of phenolic ply. There is a construction grade that is used for concrete forms, and then the grade woodworkers refer to - essentially high-quality baltic birch plywood with phenolic surfaces.
Woodcraft and Rockler both sell 1/4-sheet or smaller pieces from time to time, but don't always have it in stock. Full sheets will likely need to be special ordered and shipped by truck.
What company makes it? Maybe they can tell me who carries it around here.

phone book vs. Google
Janet, I'd just call your local lumber yards who stock quality plywood. They may not stock Phenolic ply, since it's somewhat of a specialty item, but they may be able to special order it for you through their supply chain.
In contrast, a Google search on "plywood manufacturers phenolic" turned up a long list of links. One supplier in Ohio listed phenolic surfaced Baltic birch ply, but full sheets were for local pick up only, except for a minimum of 10 sheets ($98 ea.) plus trucking (upwards of $250, they said).
In another post in the "General" section here, the poster mentioned that Anderson Plywood in CA would ship five sheets to him in Texas, with similar trucking costs.
