Installed hardwood floors and finished with 2 coats of quick drying polyurethane. After bubbles appeared in first coat, I sanded and mixed the poly with mineral spirits for the second coat. Bubbles were gone, but now a few weeks later I am finding that the poly has failed in several spots all the way down to the stain. Why is this happening and what can I do to fix/stop it?

Was it a poly formulated for floors???
Water or oil based poly? Water or oil based stain? What exactly is in the mineral spirits? The formulation has changed in the last several years.
