Table Saw Blade Wobbling REALLY BAD

Might be as simple as debris trapped between the blade and the flange.
Might be a bent blade.
If this is a direct-drive saw, the motor bearings might need replacing.
If this is a belt-drive saw, the arbor bearings might need replacing.
If it was mine, the first thing I'd do is remove the blade and wipe down the threads and the flange. Then install a different blade and see what happens.

The only thing I would add is to do one step and then the other. That way you know what caused the problem.
Are you familiar with the saw's history and do you know if it ever ran straight? Did this happen suddenly? Did the saw blade hit something?
Yes. Remove the blade, clean everything, and then install a different blade. Lay the old one on the saw top. If it is badly warped, it won't lay flat.
Closely examine the arbor washers and nut. If they are damaged, they could be forcing the blade to wobble.
While the blade is off, examine the arbor. Remove the washers and turn on the saw and watch to see if the arbor runs true.
More than likely the blade is warped. Put another blade on it. If that blade does the same thing, the arbor spindle is bent.
If it is the blade toss it.
