Does anyone make highly regarded aftermarket tool rests with a uniform top rail, allowing tied underhand grip across the entire length? My Powermatic lathe has the old style tool rest that is only thin enough for tied underhand grip at the ends.

Have you looked at the modular tool rests, like Sorby's and others?
I would strongly prefer one piece welded tool rests. Here is a diagram from Mike Darlow's Fundamentals of Woodturning showing the recommended profile.
