I recently made a zero-clearance throat plate for my Delta table saw, but after I raised the blade through the blank, I noticed that the hole made by the saw was not a straight line, but rather an arc with the center about 1/16″ farther to the left than either end. What’s the cause of this, and is there a simple fix?

I just went out and measured the slot - out of line by 1/32", not 1/16".

There are a number of possibilities:
Blade is not flat (manufacturing defect or saw overheated), or is improperly sharpened; try putting another blade on saw.
Blade flexed , or the throat plate moved while cutting.
Arbor or flange are bent. Check with dial indicator.
Wouldn't recommend doing any cutting until this problem is resolved.

a warped or flexing blade will not make an arc shape, it will just make the slot wider...i think the insert moved.

I also think the insert moved. A blade can warp, but that is not usual. I tried to cut out a blank from an old Freud for a rivving knife with a hack-saw. Got about 1/2" in 30 minutes of sawing with a new hack-saw blade. Took it to the machine shop. The steel is incredibly hard.
Check the blade to see if it is has and arc. Probaly not or you would get vibration and wobble in a cut. The cut would be wider and torn also.

Sounds to me like the blade was not 90ยบ to the table when you raised it through the insert.
