Anyone know where I can get some plans for a simple, yet critical, ramp for a wheelchair. It’s for my Dad, who was recently paralyzed. The application is to make the patio accessible to him again. There is a drop of 4.375 inches from the patio door to the patio, which I figure will require a run of 48 to 60 inches. I was thinking it should incorporate a ledge around the bottom for roll-off protection. I could use a little help on this, as I am not much of an engineer. Thanks as always.

At least a 1:12 rise/run, but the closer to 1:20 that you get, the better (sounds like you know that somehow). Your dad won't want to have help getting in and out of his home for the rest of his life and making a safe and self navigable ramp will be one of the best things you can do to smooth his this transition to a new and difficult lifestyle. Simple 2X2's are an effective safety barrier around the edges of the ramp.
