The best, IMO, is http://www.amazon.com/Swedish-Carving-Techniques-International-Classic/dp/0942391454/ref=sr_1_1?ie=UTF8&s=books&qid=1198433753&sr=8-1
I have made some from the patterns there as well as scaling them up to larger size. It is a finely engineered spoon optimizing the strength of the wood and creating a beautiful thing. The book can teach more about sloyd knife technique that you ever thought existed.
I'm still kicking myself for not buying up the dozen or so copies when Woodcraft was liquidating them for $2 each. Sadly out of print, but maybe your library has a copy.
These techniques were the subject of a FWW article back in the B&W days (pre #45) by Drew Langsner. IIRC, Drew also has some discussion of these in his books.
You might also find something similar by searching for "Treenware."
i had to make a matching fork and spoon in woodshop in school. to make them identical i used a piece of wood large enough to get both pieces out of back to back. i made a paper pattern top view and side view that i traced onto the wood. i then cut the side view with the band saw leaving a small amount still attached so i could then cut the front view out. i then split the rest apart and using wood rasp and chisels i carved the pair out and sanded both to match.
